Annual Report
In accordance with the School Regulations § 37 subparagraph 2. subpoint 2 The prerequisite for passing the year is: 2) submitting the annual report of a doctoral student referred to in section 3, together with the opinion of supervisor(s), within a deadline set by the Director.

Important report submission recommendations:
- Each report must be approved and signed by supervisors.
- In connection with cooperation with two supervisors, it is possible to submit a document in the form where the last page (with signatures) will be in three variants – each with a separate signature of the doctoral student and supervisors.
- The document must be sent in the form of a single file, i. A doctoral student is obliged to combine the reports and opinions signed by the supervisors into a single file.
- When sending the report, please write in the title of the email to the school address (szkola.m@uw.edu.pl) where the documents are transferred: Annual report – name of a doctoral student – year of education.
- A pasted scan of the signature is not valid. You can send a scanned document, or a document electronically signed with a qualified signature.
